What Is ABPM Clinical Informatics Certification?
The ABPM Clinical Informatics subspecialty certification is a credential for licensed physicians who work at the intersection of healthcare and health information technology. Diplomates use their knowledge to design clinical decision support tools, evaluate electronic health records, govern health data, and lead quality-improvement initiatives. The credential is jointly administered by the ABPM alongside other ABMS member boards, making it one of the few subspecialties open to physicians from a wide range of primary specialties.
Who Is Eligible?
Eligibility centres on two requirements:
- Primary board certification. Applicants must hold a current, valid certification from an ABMS member board, the American Osteopathic Association (AOA), the Royal College of Physicians and Surgeons of Canada (RCPSC), or the College of Family Physicians of Canada (CFPC).
- Specialty training. Historically there have been two pathways: a fellowship pathway (completion of an ACGME-accredited Clinical Informatics fellowship) and a practice pathway (for experienced physicians without formal fellowship training). The availability and requirements of each pathway have changed over the exam cycles.

How Registration Works (High Level)
The ABPM opens its application window once per year. The general steps are:
- Check eligibility using the ABPM eligibility requirements table before anything else.
- Submit your application through the ABPM online portal during the open application window. You will document your training pathway and provide supporting materials.
- Receive confirmation and, once approved, schedule your exam at a Prometric testing centre.
- Sit the exam — a 200-question, computer-based test administered in four 50-question blocks over 4.5 hours.

What Does the Exam Test?
The exam follows a published content outline with four major domains:
- Health Information Systems (40%) — EHR architecture, interoperability standards (HL7, FHIR), system selection and implementation, privacy and security.
- Clinical Decision Making and Care Process Improvement (30%) — clinical decision support design, workflow analysis, quality measurement, patient safety.
- Leading and Managing Change (20%) — stakeholder engagement, project management, change management, training, and governance.
- Fundamentals (10%) — biomedical informatics vocabulary, research methods, legal and regulatory landscape.
